Does anyone have experience at writing / managing PDP’s?

I am currently writing new Personal Development Plans for Call Centre Agents. They need to be generic to cross a number of performance objectives / products / business units. Team Managers will deliver it so I'll also be managing the process. Does anyone have any experience / materials / templates they would wish to share with me?
