Do you have a Leadership Skills/Styles Self Analysis Tool?

Does anyone have a relatively short self assessment questionnaire on Leadership Styles/Skills, for use on an open programme, that they’re willing to share please?
As well as comparing various leadership models/frameworks, I’d like to give delegates some form of self analysis that creates discussion but isn’t a large 360/MBTI type exercise. I also want to avoid a questionnaire that’s actually about management ability rather than leadership, which is the case with a few I’ve seen so far.
