What were your objectives for introducing coaching?

Not sure you can use someone elses survey – your questions will depend on what you want to measure or discover. What were your objectives for introducing coaching? What were the strategic objectives? What business needs were you meeting? What measures of success did you identify prior to the program? These are all things you should be gathering data on.
You may also want to explore the coaching process in terms of what worked for participants and what didn’t.
