You may want to try http://www.abctrainingsolutions.biz. In the freebie section (page 1 of 5) there are a number of downloadable forms, particularly Action Plan Form, Course Attendance Form, Course Enrolment Form, Course Evaluation Forms. On page 3, there is a Pre and Post Course Checklist, and on page 5 a Training Planner. All are ‘tried and tested’ and can be adapted to your needs.
