Looking for a hard copy / rather than online, self assessment questionnaire on an individuals creativity and innovation as pre-work for an up coming workshop.
Wondered if anyone had any recommendations? or willing to share?

Hello Lee – The nearest I have got to this is a questionnaire based around Geoffrey Petty's ICEDIP Model: Inspiration, Evaluation, Clarification, Distillation, Incubation, Perspiration
If you put ‘ICEDIP questionnaire’ into Google I think you should find a downloadable version of a simple form that you might use. Because of the nature of the questionnaire you may need to flex your lesson plan and content to sit alsongside the questionnaire's premis?
