Fault tree analysis

I have been asked by a colleague to help in researching a range of data gathering tools and methods that could be used in either TNA or evaluation work. Does anyone have any persoanl experience of ‘Fault tree analysis’? It may be known by another name or title. Though I’m interested in the theory of the method, I’m particularly interested in having first hand accounts of its use.
