I don’t think a standard form is feasible
Hullo Elaine. If you don’t mind my saying so I think that measuring the impact of training is rather too large an issue to be dealt with on a standardised form!
It depends on your situation or context. I guess a starting point might sensibly be “impact in terms of what?” That is, what was the training aimed at achieving? This requires detailed analysis and planning, and any attempt at impact measurement will need to be built upon this starting point. Related to this – “impact as perceived by whom?” Who are the stakeholders in the training, whose lives will be affected by it, who have a legitimate interest in its effects?
I find that the work of jack Phillips provides a lot of ideas for designing your own impact analysis – for example Return on Investment in Training and Performance Improvement Programs, Butterworth-Heinemann 2003.
A number of specialists are rather critical of Phillips – for example Paul Kearns, a regular contributor to Training Zone. You can find many pieces by him by going to Who’s Who. He has written a bestseller – Training Evaluation and ROI: How to Develop Value-based Training, CIPD 2005.

I have been researching in a simlar vein, and have found “Implementing Training Scorecards In Action”, also by Jack Phillips, with Lynn Schmidt to be quite useful. It recounts a range of case studies where a training scorecard approach has been applied.
