I think this goes beyond a training issue. The literature on Risk assessment in Mental Health is growing immensely. It’s easy to set up a form but ther whole process needs to be robust, and defensible. For starters check out http://www.ramas.co.uk/
I suggest that you might benefit from discussion with mental health colleagues in the NHS, perhaps in forensic services.
