My company is wanting to implement some major changes and as part of the project I have been asked to design a template to send out to all offices which can be used to capture what they do, when they do it, any bottlenecks, frustrations etc etc also if they have any suggestions for improvements.
Has anyone already designed something similar they wouldn't mind sharing with me.

To re arrange the old saw I think you may be looking for a rubber hammer to crack a granite block!
I’ve never come across a template to be sent out for this task…my experience (with organisations such as Hilton Hotels, Lucas Aerospace, GMAC RFC, Lloyd’s TSB Bank and TfL)is that you need to get groups together and facilitate the production of process maps. From these they can do the Process Improvement Plannng and the Downstream Impact Analyisis.
These activities, carried out to appropriate levels of detail, fulfil all the functions you require, as well as ensuring that the people themselves retain ownership of the input and the outcome, ie the changes.
Sorry that this isn’t the simple answer you were looking for….probably the most lasting and cost efficient way to fulfil the requirement is to train selected facilitators from the business in the completion of this task and then let them loose in the departments.

I have an Excel based worklog that I run at all times, making it easy to log when I change tasks. Mine is – obviously – customised to capture the information and monitoring I need – but I’d be happy to email you a copy with examples to show how I use it. Other members of my team have already adapted it for their own work and are finding really useful and easy to use.
