I am going to introduce 180° feedback into my team and would like to know whether anyone has a template of questions that they currently use and also, how effective and actionable the feedback is that you receive.

What does your team think of “you”?
Hi Sarah,
I use a free tool (actually it’s an Excel spreadsheet) that should help you get such feedback from the team.
Each month you ask the team members to rate a set of statements (using the spreadsheet) which relate to “things people need in order to do a good job”.
I use it and it’s fascinating to see how your efforts to improve the working environment – eg by introducing better and more regular performance reviews – affect the ratings you get on a month-by-month basis. Let something “slip” and you soon know about it. My aim is to keep increasing the average, and certainly never to drop below 70%.
I hope this helps with getting usable feedback. Go to this page then scroll down to ‘free stuff’: http://www.thefourthlevel.com/pages/improve_at/managing_people.htm
All the best,
John Salt.
