Action plans

Very basic request, I know, but I am looking for a standard action plan document for delegates to complete as a result of attending a training event. I am looking for a working document that details what will be changed, how it will be changed, by when, etc. If you have any such document and you can help me out, please get in touch.

  1. Personal action plan
    A basic one, based on the points in your message, would be three columns with the following headings:

    Personal goals/intended achievements (can be short/medium/long term)

    Strategies and resources (how goals will be achieved)

    Review date/date achieved

    We use this with nursing students during clinical placements and it works well, although we call it a learning contract.

  2. Simple action plans
    I sometimes use a very simple, direct action plan at the end of events:

    What I’m going to STOP doing
    What I’m going to START doing
    and what I’m going to continue doing (because this course has confirmed I’m doing it well)

    Include dates for action and any support needed (eg resources, manager’s input, etc).

    I’ve used one similar to Jennifer except I call it “Keep”, “Chuck” and “Create” just because I think it sounds a bit more positive than ‘start’ and ‘stop’. It also works really well for team / group planning

    If I’m doing more traditional goal setting ones, I also ensure that participants include a column relating to what help they need and who they will approach to get that support – as we rarely achieve change goals by ourselves.
