Im after any process steps people may have on conducting a Deepdive analysis. I have a broad overview but would like some advice from anyone that may have done one.
I intend to do a Deepdive on my organisations Risk Managment System and use the actual analysis process as a learning activity to look at the Risk Management System. I will then feed the analysis finding back into the business as a continuous improvement activity.
Any help would be appreciated.

No answers posted – I'd tend to go down the '5 whys' (root cause) route, with its associated tools (fishbone etc). haven't seen anything specifically labelled 'deep-dive' (a more recent bit of jargon that's creeping in).
