I have come across an excellent 'Blockages Survey' but it's not attributed. The headings on the score sheet are Self-Management, Values, Personal Goals, Personal Development, Problem Solving, Creativity, Influence, Managerial Insight, Supervisory Skills, Trainer Capability and Team Building Capacity. Can anyone help me out here please?

The Unblocked Manager – Mike Woodcock
It is “The Unblocked Manager” by Mike Woodcock. I have not used this one personally, but it is available from Amazon etc.
