Coaching For Performance Improvement

Dear All,

I am a master degree student and I need to perform a research about the relationship between coaching and performance improvement of employee.

To do so, I am planning to create a survey to gather the data, however I need support to create the questions which will direct me to measure the connection.

Would it be possible for you to help me on my survey creation?

Sample questions, previosly made researches, techniques to gather the data, measurements, evaluation sytyle information is appreciated.
