customer service questionnaires

can anyone point me in the direction of, or has anyone got any customer service questiionnaires. This is to evaluate customer relationship management, not the evaluation of the training received.
any help would be appreicated

  1. Identify what constitutes an excellent customer experience for y
    All we ask at the moment is how they rate the course administration, on a scale from 1 to 5, from very poor to excellent, with space for comments, and whether the environment was conducive to learning. You could identify what constitutes an excellent customer experience for your trainees then construct questions and ratings to obtain trainees’ feedback. For example, appropriate and sufficient pre-course information about the training, feeling welcome on arrival, telephone/e-mail/postal enquiries dealt with efficiently, access to satisfactory beverages and food, special dietary needs catered for, comfortable training environment, provision of facilities/help for persons with a disability or special learning need, perceiving that they are valued and respected, suggestions for how the customer experience could be improved etcetera.
