I have details of a bacie leadership style questionnaire which I understand is based on situational leadership.

Situational Leadership Questionnaire
I have recently used a questionnaire by the Ken Blanchard organisation (of Hersey and Blanchard fame)which is highly effective. It comprises a self-report questionnaire, a self-scoring form and what is known as the ‘Colour Model’. This will cost about £8-£10 per person and is the basis for a half day seminar. It encourages useful debate on leadership styles.

I used to get all the various questionnaires from Pfeiffer & Co. Also if you have access to the old University Associates training materials and exercises books, there was a version in there.
