Learning organisations should probably not be repeating costly mistakes(!), so how about a measure that relates how many major mistakes are repeated, and the costs/consequences of these?

Not sure if it’s what your looking for (and I haven’t read it in detail) but chapter 8 of Peter Senge’s book The Dance of Change – The challenges of sustaining momentum in a Learning Organisation, migh be worth a look at. The chapeter is entitled Assessment and Measurement.

Elizabeth, Whilst it doesn’t give the definitive answer, Peter Senge’s ‘The Fifth Discipline’ does give a comprehensive insight into learning organisations and how to create them.

Adding to other recommendations here, Senge’s “The Fifth Discipline Fieldbook” is also worth consulting, as is the ASTD Infoline “16 Steps To Becoming A Learning Organization” (Marquardt).
You should be able to download the Infoline from the ASTD website bookstore.
