   Apparently it is the central theme of the “Harvard Policy Model”, developed at Harvard Business School in the early 1920s.

   I know what you mean: someone must have thought of it, unless they sat round in a meeting and four different people each came up with one of the letters!

   But I’m afraid I’ve found very little on the origins on the web apart from the below. Perhaps you could contact Harvard, if it is very important to know?
